When a user uploads photos to the review and adds a caption, the caption is attached to the wrong image.
(See the product reviews in the link attached.)

The photo that clearly shows a restored chair has a caption “before” and the old chair has the caption ‘After!!!’ . The user who submitted the review complained that it was wrong, which is how we learned about it.

I tested and can confirm that captions are not attached to the right image. This is reflected in the media library when you look at the image, the caption is saved incorrectly.

So I think, it could be possible that a theme or plugin conflict is causing this behavior. In this case, we would recommend testing by taking the following steps: https://docs.woocommerce.com/document/woocommerce-self-service-guide/
